Case Manager Assistant

Job Summary:

We are seeking a highly organized and proactive Case Manager Assistant to support the coordination and delivery of care within our rehabilitation programs. This role provides administrative and operational support to Case Managers, helping ensure that client needs are met efficiently and that care plans are executed accurately and on time. The ideal candidate will have experience in healthcare or social services, strong communication skills, and a compassionate, client-focused approach.

Responsibilities:

  • Assist Case Managers in coordinating services and follow-ups for patients, including scheduling appointments, confirming provider availability, and arranging transportation when needed.

  • Maintain and update client records, ensuring all documentation is accurate, complete, and compliant with healthcare regulations.

  • Serve as a point of contact for clients, providers, and internal team members to relay updates, gather information, and assist with inquiries.

  • Track care plan progress, monitor deadlines, and provide reminders to ensure timely execution of tasks.

  • Prepare reports and summaries related to client status, service utilization, and follow-up actions.

  • Help coordinate multidisciplinary team meetings and maintain related documentation.

  • Support the Case Managers with any additional administrative tasks required to streamline case coordination and delivery of care.

Requirements:

  • Associate’s or Bachelor’s degree in Healthcare Administration, Social Work, Psychology, or a related field preferred.

  • 1–2 years of experience in healthcare, case management, or administrative support roles.

  • Knowledge of medical terminology, care coordination, and HIPAA regulations.

  • Strong organizational skills and attention to detail.

  • Excellent written and verbal communication skills.

  • Proficiency with EMR systems and Microsoft Office/Google Workspace tools.

  • Ability to work independently in a fast-paced, remote environment.

  • Compassionate and professional demeanor when interacting with clients and families.
